Fournier’s gangrene is a rare, life-threatening bacterial infection of scrotum, penis or the area between genitals and rectum. This skin infection worsens quickly and requires emergency care. Genitals or perineum becomes red, tender or swollen with high grade fever. In this gangrene tissues die because of both aerobic and anaerobic bacterial infection resulting in microthrombosis of the small subcutaneous vessels. Fournier’s gangrene is a type of necrotizing fasciitis or flesh-eating disease which destroys soft tissues, including your: * Arteries (blood vessels). * Muscles. * Nerves. It shows a predilection for patients with diabetes as well as long term alcohol misuse; however, it can also affect patients with immune compromise. The nidus is usually located in the genitourinary tract, lower gastrointestinal tract, or skin. The development and progression of the gangrene is often fulminating and can rapidly cause multiple organ failure and death in 3% to 10% cases. It commonly affects men, but can also occur in women and children. Plain X ray may show air within the tissues. Ultrasonography is useful to differentiate intrascrotal abnormality. FG needs an aggressive multimodal approach, which includes haemodynamic stabilisation, broad spectrum antibiotics, and early multiple staged surgical debridements. Even though testes are classically spared in the process of FG, orchidectomy, for non‐viable testis, is eventually required in up to 21% patients. Split thickness skin graft is the treatment of choice in treating perineal and scrotal skin defects. Meshed slits are oriented transversely without expansion. Graft take, appearance, and sexual and voiding function are excellent. Erectile function and ejaculation are preserved in potent patients. Use of vacuum assisted closure (VAC) system dressing has improvement with minimising skin defects and speeding tissue healing.
