Annual Review

All active research protocols involving biological hazards, regardless of funding status or source, are reviewed by a member or members of the IBC at least annually.

If the Principal Investigator fails to submit a continuing review application by the anniversary date, a cease and desist letter will be sent to the Principal Investigator. All activities described in the expired protocol must cease.

Timing: Investigators must submit and receive approval prior to the expiration date.

Forms

Sixty days before your expiration date, you will receive a notice that your Continuing Review is due. At the end of the notice you will find a link to your continuing review submission form. Click this link or copy the link into your internet browser to complete the continuing review form.
