The medical evaluation must be performed by a medical doctor (MD/DO) or a nurse practitioner (NP). We CANNOT accept evaluations performed by physician’s assistants (PAs). Some doctors and nurse practitioners may use specialized titles such as FNP-C for Certified Family Nurse Practitioner. Please be sure to get the evaluating physician’s title prior to receiving your evaluation and contact Dive Operations if you have any questions.
Unfortunately, there are no known doctors in Corpus Christi with experience in diving and hyperbaric medicine. Any medical doctor (MD/DO) or nurse practitioner (NP) can perform the evaluation whether it is a family doctor, current evaluating physician, or from an urgent care clinic. There are several walk-in clinics that can perform the evaluation, however, please coordinate with Dive Operations on the day of your evaluation as some clinics may only staff Physician’s Assistants (PAs) who’s evaluations cannot be accepted by the TAMU-CC Dive Program.
TAMU-CC provided insurance is accepted by most general practitioners and some walk-in clinics. Please check with your specific medical evaluator to ensure that your insurance will be accepted for every part of the medical evaluation, as some tests may not be covered. TAMU-CC insurance requires an annual wellness exam. Contact your evaluating physician to see if it is possible to complete the annual wellness exam and the medical evaluation for diving in the same session. If you carry non-TAMU-CC insurance, please check with your specific doctor’s office to ensure coverage.
FAQ: Operational Dive Log
Please submit dive logs within 4 weeks of the actual dive dates. The dive logs are the record to track required minimum dive activity. The earlier they are submitted, the earlier your dives and training records are updated.
The Lead Diver is responsible for ensuring that the dive log is completely filled out. If the Lead Diver is diving or supervising another part of the dive operation then it can be delegated to the topside personnel overseeing diver check-ins and check-outs.
FAQ: Letter of Reciprocity
If you plan to dive with another scientific diving institute, a Letter of Reciprocity (LOR) can be sent to the institute that you plan to dive with by completing the online form above. The LOR contains the necessary forms to verify your current diver status and training record.
Please check several sources of local forecasting prior to, and the day of, your dive.
Please submit your dive plan for review at least 1-2 weeks prior to your dive. If you are diving in a new dive site or on a new project, you should submit 3-4 weeks in advance in case changes to your dive operation are required.
FAQ: Dive Accident Report
Any and all injuries should be reported including lacerations and other minor injuries. An accident report should be submitted as soon as possible to ensure workman’s compensation coverage.
